About teeth whitening
Teeth darken in two ways. Extrinsic stain sits on the enamel surface, picked up from coffee, tea, red wine, and tobacco; much of it comes off during a [professional cleaning](/services/teeth-cleaning/). Intrinsic discoloration is inside the tooth — the natural yellowing of dentin as enamel thins with age, or staining from certain medications during tooth development.
Whitening gels work on intrinsic color. Peroxide penetrates the enamel and breaks apart the pigmented molecules held within the tooth structure. Concentration and contact time determine the result, which is why a supervised course produces more change than a drugstore strip. It is also why supervision matters for comfort — sensitivity is dose related and manageable when someone is adjusting the protocol.

In-office versus take-home
In-office whitening uses a higher-concentration gel applied under supervision with your gums protected. It produces visible change in a single appointment, which is what you want before a wedding or an event.
Custom-tray whitening uses trays made from a scan of your teeth and a lower-concentration gel worn at home over about two weeks. The result is comparable, it costs less, and you keep the trays for touch-ups.
Combination — an in-office session for immediate change, then trays to extend and maintain it — is what many patients choose.
Why the exam comes first
Whitening gel reaches whatever the enamel exposes it to. If there is an untreated cavity, a leaking filling, an exposed root surface from gum recession, or a crack, the peroxide reaches nerve tissue directly and it hurts.
This is the actual difference between professional and over-the-counter whitening. The gel chemistry is similar; the examination is not. Ten minutes of looking prevents the outcome where someone spends two weeks in pain and concludes their teeth are “too sensitive to whiten.”
Keeping the result
Whitening is not permanent — teeth re-stain. What extends it:
- Rinse with water after coffee, tea, or red wine
- Use a straw for iced coffee and dark drinks
- Keep up professional cleanings, which remove new surface stain
- Do a one- or two-night touch-up with your trays when you notice drift

How long do whitening results last?+
Typically one to three years, depending on diet and habits. Coffee, tea, red wine, and tobacco all shorten it. Keeping your custom trays lets you do a short touch-up rather than a full course.
Does whitening damage enamel?+
Professionally supervised whitening at appropriate concentrations does not damage enamel. The main side effect is temporary sensitivity, which resolves after treatment. Problems come from unsupervised overuse of high-concentration products.
Why are my teeth sensitive during whitening?+
Peroxide temporarily increases fluid movement through the tubules in dentin. It is common, it is temporary, and it is manageable — we can reduce wear time, lower the concentration, or add a desensitizing gel.
Will whitening work on my crowns and fillings?+
No. Porcelain and composite do not change color. If you have visible restorations on front teeth, we whiten your natural teeth first, wait for the shade to stabilize, and then replace those restorations to match.
In-office or take-home — which is better?+
In-office is faster and useful before an event. Take-home custom trays generally produce equally good results over a couple of weeks, cost less, and leave you with trays for future touch-ups. Many patients do both.
Are drugstore whitening strips worth it?+
They produce modest improvement on mild staining. They do not adapt to your teeth, so coverage is uneven around the edges, and there is no examination beforehand — which is the real risk if you have an untreated cavity.
Can everyone whiten their teeth?+
Most people can. Whitening works less well on grey discoloration from tetracycline or trauma, and it is not appropriate over untreated decay or significant gum recession. We will tell you honestly what result to expect before you spend anything.
When should I whiten if I am planning other cosmetic work?+
Always before. New restorations are matched to your teeth, so the shade has to be settled first — otherwise your new crown or veneer will be the wrong color once you whiten later.
